Surgeons-ophthalmologists from all over Russia, from Great Britain, USA, Saudi Arabia, Egypt, Switzerland, Germany, Italy, France, Sweden, neighboring countries, are traditionally collected for the conference experience. Specialists are discussing new technologies implemented in clinical practice. This forum is one of the largest in Russia, it is held once every three years.

REVIEW
VIII Euro Asian Conference on Ophthalmosurgery

A large scientific international event was the holding C 26 to 28 April 2018 In Ekaterinburg VIII Euro Asian Conference on Ophthalmosurgery (VIII EAO) . The history of this event originates back in 1998. The name of the conference was not accidentally given, it was on the border of Europe and Asia, to the city of Yekaterinburg, every three years, ophthalmologists come from different parts of our country, as well as far and neighboring countries. As expected, the final figure of the participants in the conference passed for a thousand. For 20 years, the EAO is a unique innovative and discussion platform, where ophthalmologists have the opportunity to share their experiences and take over the operations of their colleagues. The Ekaterinburg Eyes Microsurgery Center (Center) remains a permanent organizer and ideological inspirer of this forum. This year, VIII EAO opened its doors in the Center for International Trade Yekaterinburg (CMTU). For three days, in the six halls, the discussion of the discussion issues of modern ophthalmosurgery was rapidly held.

April 26, 2018. First day VIII EAO.

The day began with the fact that the General Director of the Center, the main ophthalmologist of the Sverdlovsk region, Chairman of the Organizing Committee of the EAO Oleg Vladimirovich Shilovsky The medical exhibition was solemnly opened at which the latest medical equipment was presented in modern ophthalmosurgery. The largest world manufacturers offered their development universal attention, which are already used in a number of domestic and foreign clinics, as well as what is still at the stage of testing, what is planned to be implemented in a wide range of ophthalmic equipment. For example, surgical equipment with a 3D system, which received good recommendations in the United States and is currently entering the market in Russia.

opening of the exhibition 3D

A little later, in the atmosphere of the holiday and celebration, the opening of the VIII Euro Asian Conference on Ophthalmosurgery, which was attended by "high" guests.Chairman of the Legislative Assembly of the Sverdlovsk Region Lyudmila Valentinovna Babushkin It rightly noted that this year the Yekaterinburg Center of the MNTK "Microsurgery Eyes" will celebrate its 30th anniversary: "During this time, the technology has gone far ahead, so it is important to continue this kind tradition – to hold conferences of this level. Regional authorities, for their part, within the framework of public-private partnerships support this center, because the treatment of eye disease remains an urgent problem for the inhabitants of our region. A couple of dozen years ago, the excitement was aroused by everything that concerned ophthalmosurgery, and now there was an absolute confidence in the MNTK microsurgery doctors.

Deputy Minister of Health of the Sverdlovsk Region Sergey Borisovich Turkov: "I am glad that it was in Yekaterinburg that such a discussion platform is organized, where specialists in the field of ophthalmology can share their experience, discuss the exciting questions"

Also at the opening attended a special guest from Moscow, corresponding member of the Academy of Sciences of the Russian Federation, Professor Hristo Periclovich Tahchidi. He recalled why everything began and how the idea was born to organize with us, in Yekaterinburg, the event of this level.

Opening of the EAO Img_0387.jpg Live Surgery

The most long-awaited and impressive stage of the conference is "Living Surgery", where the entire spectrum of surgical activity was presented. The broadcasts were carried out from the operating blocks of the center of refractive laser surgery and the main building of the Ekaterinburg Center of the MNTK "Microsurgery of the eye". A very important achievement of the VIII EAO has become a high quality of image transmission. Many noted that for the first time in Russia, with the help of digital technologies, it was possible to achieve such a clear picture on the big screen. In the non-stop mode, live surgery was broadcast, switching from one operblock was replaced by a show of the next non-standard case.

Traditionally, innovations were presented in the field of ophthalmoshurgery, but the distinctive feature of the section was that the developers of surgical techniques and methodologies showed them in their copyright.

To the attention of the conference participants was presented 10 patients with various pathologies. It was important to show operations with specific diseases. A crowded room with interest was followed as ophthalmoshurgeh, relying on personal practice, shared by their skill, "out" from difficult cases. At the same time, the audience had the opportunity to ask any exciting questions and receive advice and recommendations from the "live" surgeons operating

The moderator of the section performed Boris Vladimirovich Laptev (Yekaterinburg Center MNTK "Microsurgery Eyes"). The presidium of the section includes: Alexey Nikolaevich Ulyanov (Yekaterinburg Center MNTK "Microsurgery Eyes"), Zafar Umarovich Sidikov (Uzbekistan), Vladimir Vladimirovich Kapralov (Germany).

Leading ophthalmologist of Europe, professor Walter Secundo (Germany) demonstrated the author's technique of the SMILE operation (femtosecond technology of refractive surgery) developed by him. Today it is the most advanced laser technology for vision correction. Of great value and novelty is the fact that it is the features of the author's performance that make it possible to see the specifics of his methodology, to feel his approaches to this operation.

But even more interesting is the fact that the same technology, but using our own approaches, was demonstrated by the Deputy General Director of the Yekaterinburg Center of the IRTC "Eye Microsurgery" Sergei Viktorovich Rebrikov. He showed how the Center's ophthalmic surgeons continue to develop this surgical method. Thus, the participants of the conference had the opportunity to compare different techniques for performing the same surgical method and make sure that they are being developed on the basis of the Center.

Chairman of the Society of Ophthalmologists of Russia, Professor Boris Eduardovich Malyugin (Moscow), the author and developer of a device that allows to achieve persistent pupil dilation during lens surgery (a quadrangular pupil retractor or Malyugin's ring), operated on a patient with cataracts and astigmatism by implanting an enVista TORIC toric lens. As you know, a narrow pupil causes difficulties during any surgery and increases the risk of complications during the operation. Interest in this operation was that with astigmatism it is important to correctly install the intraocular lens (IOL) in a certain position, which is also difficult with a narrow pupil. The author demonstrated how using the "Malyugin's ring" it is possible to successfully operate on patients with cataracts and astigmatism with a narrow pupil, achieving maximum visual acuity in the postoperative period.

Img_3561.jpg

Another approach in the tactics of rehabilitation of patients with cataract and astigmatism was demonstrated by the President of the Russian Society of Cataract and Refractive Surgeons, Professor Vladimir Nikolaevich Trubilin (Moscow). It is noteworthy that in this case the operation was carried out in two stages. The first stage was the correction of astigmatism by applying corrective incisions, using a LenSx femtosecond laser, which was performed by the debutant of "live surgery" Igor Sergeevich Rebrikov (Yekaterinburg Center of the IRTC "Eye Microsurgery"). Further, Professor V.N. began the main stage of the operation. Trubilin and, demonstrating all the nuances of his precise, verified technique, successfully removed the cataract and implanted the PanOptix multifocal lens.

MD Dmitry Ivanovich Ivanov (Ekaterinburg Center of the Moscow Scientific and Technical Center "Eye Microsurgery") presented the author's technology of trabeculotomy in cataract surgery. For many years, the technique developed by him has been successfully used on the basis of our Center and has become widespread both among ophthalmologists in Russia and around the world. This was also noted by Dr. V.V. Kapralov (Germany), who took an active part in the discussion during the broadcast of this operation.

It is a matter of pride that for the first time in the Ural Federal District, on site VIII of the EAKO, the latest achievements in ophthalmic surgery were demonstrated, in particular 3D technology – the NGENUITY system, which allows operating on the pathology of the vitreous body and retina without using an operating microscope in a stereoscopic image. This system appeared in Russia recently, and the first demonstration of its capabilities took place during the "Live Surgery". What was previously inaccessible to ophthalmic surgeons is now achievable. The possibilities of visualizing the details of the retina have increased due to the use of modern digital image transmission technologies. Demonstrate the capabilities of new technology Vincenzo Ferrara (Italy) and MD. Viktor Nikolaevich Kazaikin (Yekaterinburg Center of the IRTC "Eye Microsurgery"). They noted that the NGENUITY system provides a comfortable position for the surgeon: during the operation, there is no need to bend over the microscope, it is enough to put on stereoscopic glasses and watch the image on the screen, receiving a three-dimensional image. All this increases the accuracy of actions and gives a high result.

Chief ophthalmologist of the Ministry of Defense of the Russian Federation Alexey Nikolaevich Kulikov (St. Petersburg) showed coagulation of ciliary processes under endoscopic control using the EndoOptiks device to reduce intraocular pressure in combined surgery with phacoemulsification and IOL implantation.

Naumov K.

We are pleased that at the VIII EAKO the composition of surgeons who “live” showed their work has “rejuvenated”. Along with titled foreign specialists, young surgeons of our Center look no less worthy. Dmitry Borisovich Bardasov and Maxim Evgenievich Nikulin (Yekaterinburg Center of the Moscow Scientific and Technical Center “Eye Microsurgery”) demonstrated operations using technologies developed on the basis of the Center, such as reposition with suturing of a dislocated IOL and restoration of the structures of the anterior segment of the eye with organic blockade of the anterior chamber angle in a patient with cataract and glaucoma.

The high level of new digital technologies made it possible to enjoy the quality of "live" surgery, to see all the nuances and details. The applause grew louder and louder in the hall after each live broadcast.

At the video section "Non-Standard Surgery" video blocks were presented to the attention of the conference participants, where the leading ophthalmic surgeons of Russia also practiced complex, non-standard cases in real time, tested the latest medical ophthalmological equipment in various types of operations.During the broadcasts, comments on the used medical devices and techniques sounded. Each conference participant had the opportunity to clarify the advantages of the presented equipment from speakers, ask any question of interest, to obtain a good advice from high-class specialists.

The "Obloplastic Surgery" section touched upon such important aspects as the restoration of tears, eye prosthetics, features of children with congenital anomalies and the other.

Satellite symposia from leading medical companies gave a more complete picture of an assortment concerning ophthalmic surgery presented in the global market.

Before space gala dinner

By the end of the first day, VIII EAO communication continued in an informal setting, on a solemn dinner, where a large cultural program was prepared for guests. All this secured the feeling of a big professional holiday.

April 27, 2018. Second day VIII EAO.

In the hall there are microphones, a business atmosphere reigns, a foreign speech is heard. The second day of the conference began in parallel with several sections: cataracts and optical and reconstructive surgery, ophthalmosesthesiology, refractive surgery.

Malyugin B.E.

In the main hall Boris Eduardovich Malyugin (Moscow) read the memorial honorable lecture on the name of the Binshorst, which he represented a year ago in America and Lisbon. The speaker applied to the story, recalling the founder of domestic ophthalmosurgery Svyatoslav Nikolayevich Fedorova.

Among the reports submitted were not unambiguous and one-bone operations, each report had a reason for the dispute. The reporting program was built in such a way that after each performance was time for discussion and discussion. The audience did not hesitate to ask questions, the format of the "live dialogue" heated interest in what is happening.

In addition, on the second day, the VIII EAO traditionally passed courses on vitreoretinal surgery. They took part: Stefan Mennel (Austria), Michael Goldbaum (USA), Vincenzo Ferrara (Italy), Shuichi Yamamoto. (Japan) and other world-class specialists. The audience of the courses numbered about 80 listeners.

S.Mnelel V. Ferrar M.Goldbaum

Courses in vitreoretinal surgery – this is a unique project, started back in 1991 by the professor Ingrid Krassssign (Germany), at the time led by the Department of Ophthalmology of the University of Tubingen. The essence of the project is that beginners (and not only) ophthalmologists can join the experience of leading world experts in this area without leaving for other countries. Over the past time, the project has covered space from Africa and Asia to Iceland. At the VIII EAO courses took place in the 128th time. Traditionally, the program is conditionally divided into two parts: the first sets out the fundamental basics of the diagnosis and surgery of the retinal detachment, the second presents modern data from some of the selected sections of the vitreoretinal pathology. At the same time, many listeners have repeatedly visited courses. In Yekaterinburg, on the site of the VIII EAO, fifth courses were held, starting in 1995. Their listeners say that every time you have the opportunity to hear something new.

Sh. Yamomoto

This time the second part of the program was devoted to the problem of age macular degeneration (NMD). In particular, issues of applying modern diagnostic methods (S. Yamamoto) and modern approaches to the treatment of "wet" form of NMD (M. Goldbaum) were covered.

As usual, in the final part of the courses, several interesting clinical cases of retinal debris and NMD were presented for discussion of experts and audience.

There were also several satellite symposia devoted to the problems of glaucoma, bioproadsts in ophthalmosurgery, modern methods of cataract surgery. These topics traditionally cause great interest in Russian, but also in foreign colleagues.

For the first time, the conference was "live" diagnosis of patients with glaucoma. With the help of modern equipment, the condition of the visual nerves of patients was estimated, and the diagnosis was made. "It is very important to reveal to glaucoma in the early stages, as it begins asymptomatic. Patients with the fourth stage of glaucoma, unfortunately, no longer help, the vision is lost forever. However, over time, various devices and equipment with new features appear, so it is very valuable at the conference to share with the ophthalmologists of the whole world with new information and experience, "says the head of the city department of diagnosis and treatment of glaucoma of the Yekaterinburg Center of Eyes Iia Georgievna Postaryova.

Img_5244.jpg

The final note of the second day VIII EAKO was the concert of the youth orchestra of the Ural State Academic Philharmonic, where guests of the conference were invited. The program of the creative evening was preparing for a year specifically for them and included well-known works of popular classical music both Russian and foreign composers (Brams, Strauss, Dunaevsky, Khachaturian and others). Beautiful music, masterly executed by the orchestra, did not leave anyone indifferent!

April 28, 2018. Third day VIII EAO.

H.P. Tahchidi

The last day of the conference began with the active work of the section on the vitrehetinal surgery, which was opened Hristo Periclovich Tahchidi The report on the proper experience of the implantation of the Argus II system in two patients with pigment retinitis (for the first time in Russia). This system has shown the possibility of transmitting artificial visual information through the preserved retinal structures in the cortical analyzer, as well as perception and recognition of the cerebral cortex of this information.

With a lecture on memory Cesare Flyni, performed Vincenzo Ferrara (Italy). All those present in the room once again plunged into the radiant smile of the Great Vitreoretinal Surgeon Cesare Flyni, who was and for all will remain in our memory a real professional, friend and man with a capital letter. The audience of the main hall Standing his memory of a minute of silence.

The day continued with the presentation of poster reports. In general, 27 poster works were sent to the EAO Organizing Committee. Of these, only three were chosen for awarding. Reports considered the competent commission, which included: Professor, D.M. Armen Benikovich Stepanyantz (Ekaterinburg), Professor, D.M. Tatyana Valeryevna Gavrilova (Perm) and Ph.D. Igor Alexandrovich Malov (Yekaterinburg).
